Seattle Prosthodontics, Dental Implants & General Dentistry • Seattle, WA
Whether you were referred to us for specific treatment, or are merely looking for a general dentist, your initial appointment will consist of an exam by Dr. Goll as well as a teeth cleaning and x-rays if applicable. Prior to discussing his findings, he spends time speaking to you, the patient, and any family members that may wish to learn more about what you are looking for in terms of results to achieve your ideal and desired smile. After getting to better know your expectations, we begin to outline the best treatment options for you, taking into consideration, medical history, age, cost and insurance coverage.
Please assist us by providing the following information at the time of your appointment if available:
- Your referral slip and any x-rays and periodontal charting if applicable (unless your dentist has sent these to us directly).
- A list of medications you are currently taking (especially those taking blood thinners or bisphosphonates).
- If you have dental insurance, bring your card and the necessary completed forms.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the first visit.
If your dentist or physician has taken x-rays, you may request that they forward them via email (firstname.lastname@example.org) or mail (see map and location for address) to our office. If there is not enough time, please pick them up and bring them to our office for your initial exam. If additional films are necessary, they can be taken at our facility.
